For all Youth / Teen Programs
New for 2023! You will receive an email from Camp Docs after you are registered for a program. Camp Docs will allow you to create a profile that will include all of the information that was asked for in the previously required documents listed. You will also be able to upload well-visits and immunization records.
- Family Record - to be filled out once per year, per family.
- Medical Authorization Form - to be filled out once per year, per child.
- PRCE Camps Swim Test Permission Form (In accordance with Christian's Law, Massachusetts General Law c.111, s 127A1/2)
- A copy of your child's Well-Visit Physical within the past 12 months turned in to the office
- A current Immunization Record turned in to the office (COVID Vaccination record if applicable)
- All required forms must be turned in for a registration to be complete!
These permission forms or waivers are still required to be completed separately of Camp Docs.
