   Please submit this form. After the LHC approves your application, we will ask you to please supply a check. The cost is $62 and is payable directly to the artisan painting your marker. If you are not sure about the history of your property, you may submit the form for LHC review or research property information on the Commonwealth of Massachusetts MACRIS website.
